Dog Adoption Contract | From Forgotten to Forever

FROM FORGOTTEN TO FOREVER (FFTF) RESCUE ADOPTION CONTRACT*

THIS ADOPTION CONTRACT is made and entered into on the below date, with From Forgotten to Forever Rescue, (hereinafter referred to as FFTF), and the below named individual (hereinafter referred to as Adopter, whether one or more persons).

I, herein referred to as Adopter, hereby agree that the above described animal is being adopted by me solely as a pet for myself and/or my immediate family. I agree that I will not sell, give away, or otherwise dispose of said animal to any person(s), dealer, retailer, auction, institute or any other entity for any reason. If at a later date I am unable or unwilling to keep this pet, I agree to first contact FFTF and give them the option to reclaim said pet at no charge. I hereby agree to care for the above described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water, and veterinary care. I further agree that said pet shall reside inside my home and shall not be allowed to roam freely. I hereby agree that this pet shall wear a collar or harness with identification tags at all times. Adopter agrees to register and update dog’s microchip information and to keep FFTF as a secondary contact. Adopter also agrees to not subject dog to medically unnecessary procedures for cosmetic purposes such as ear cropping or tail docking.

Adoption Fee*

I hereby agree that the adoption fee helps to cover FFTF’s expense of raising and caring for the animal and is not refundable after a trial period of 7 days. Upon the end of this period, this adoption shall be considered final. If above adopter retracts this adoption within the trial period, half of the adoption fee will be returned with the other half being considered a donation to FFTF.

FFTF has no knowledge of the temperament of the dog except as follows:

I hereby understand and agree that FFTF makes no representations or warranties, expressed or implied, about the above mentioned animal's temperament and is hereby absolved from any liability for future damages or injuries caused by said animal. I also understand and agree that FFTF further gives no guarantees, expressed or implied, of the suitability of the animal to the adopter and/or his family.

FFTF reserves the right to request a home visit to verify a successful placement and well-being of the dog. Should Adopter breach any of the agreements contained in this Adoption Contract, FFTF shall have the right to rescind this Adoption Contract and demand return of this dog. The Adoption fee shall not be refunded. Should Adopter fail to return dog FFTF may seek an Order from a Court of competent jurisdiction for the return of said dog and liquidated damages from Adopter to any sum paid for this dog pursuant to this Adoption Contract plus attorney's fees and court costs.

The undersigned specifically acknowledges that his/her signature below signifies his/her authority to bind all members of the household to the terms of this agreement. Unless otherwise stated herein, all the provisions of this Agreement shall be binding upon the respective heirs, estates, next of kin executors, administrators, and assigns of the parties. The adopter understands that this dog may have been exposed to kennel cough or upper respiratory infection, parvo, parasites, or other illnesses prior to adoption. Adopter understands these symptoms may take 7-14 days to appear. In the event of illness, FFTF may authorize care from an FFTF appointed veterinarian. Should adopter choose to take animal for veterinary care without consent of FFTF, the adopter agrees that this is at will and at own expense.

I certify that all statements made by me on this adoption agreement are true and correct. I agree that the current owner has the right to confiscate the above described animal in the event that any statements made by me are found to be false and/or my check for the adoption fee is returned for insufficient funds.
